Please note that the Health Department cannot replace lost/missing vaccine cards.

Before scheduling your vaccine, please read through the following information carefully to make sure that your appointment is scheduled successfully.

Unless you receive a confirmation number via email or text message, your appointment time is not valid.

If you would like to pre-register at any of the walk-in clinics, please click on the date you would like to attend. If you do pre-register, bring the confirmation email/bar code that you received. If you do not pre-register, be sure to bring an ID.

WCHD does not currently have J&J vaccine in stock. 

  • Wednesday, September 22, 2021 9:00 AM - 11:00 AM
    • Pfizer and Moderna will be offered at this clinic
    • If you are receiving 2nd dose of Moderna, 1st dose must be on or before August 25, 2021
    • If you are receiving 2nd dose of Pfizer, 1st dose must be on or before September 1, 2021
  • Wednesday, September 29, 2021 9:00 AM - 11:00 AM
    • Pfizer and Moderna will be offered at this clinic
    • If you are receiving 2nd dose of Moderna, 1st dose must be on or before September 1, 2021
    • If you are receiving 2nd dose of Pfizer, 1st dose must be on or before September 8, 2021

You must bring your vaccine record card with you to have second dose information added. Make sure you print or have access to your appointment ticket/bar code if you preregister.

Please wear short sleeves to the clinic to aid in the vaccination process. A face mask is required. You will need to wait at least 15 minutes after receiving your vaccine for monitoring.
